With our community facing the biggest public health challenge of its lifetime, we recognise that for many this is a difficult time, and that your health, livelihood and connections may have been affected.

The “Together in Nillumbik” survey is being conducted to better understand how the community has been impacted by the COVID-19 pandemic and to help us determine what further support is required.

We have partnered with healthAbility Community Health Service to deliver the survey, which will inform how Council and local service providers can best address your changing health and wellbeing needs during COVID-19 Pandemic and beyond.

To compliment the survey Council in collaboration with healthAbility Community Health Service and Diamond Valley Community Support, has established a “Together in Nillumbik: Local Services Network”, which includes cross service sector representation, working together to keep our community thriving during this challenging time.

Nillumbik Shire Council and HealthAbility are collecting your information for the purpose of identifying community needs during the COVD-19 pandemic.

The information you provide will be held in accordance with the Privacy and Data Protection Act 2014 and the Health Records Act 2001. De-identified responses will be shared with third party community health services for them to understand and respond to community needs.

You are not required to be a registered user of this site to submit your survey response.
